Your Right to a Good Faith Estimate

At Centered Connections, you have the right to receive a Good Faith Estimate explaining how much your care will cost before you begin.

Centered Connections is a private-pay practice. We do not bill insurance, so every client pays for services directly. For that reason, we provide a Good Faith Estimate of expected charges to each client before services begin, whether or not you have insurance.

Under the Federal No Surprises Act

What the law guarantees you

  • You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency service, including a comprehensive assessment and any related items.
  • We will provide your Good Faith Estimate in writing after you schedule a service, and within three business days when you request one. You may also request an estimate before you schedule.
  • If you are billed at least $400 more than your Good Faith Estimate, you have the right to dispute the bill through the federal Patient-Provider Dispute Resolution process. You must start within 120 calendar days of the date on your bill, and starting a dispute will not affect the quality of your care.
  • Keep a copy of your Good Faith Estimate in a safe place.

If you have insurance and would like to seek reimbursement, we can provide a superbill upon request that you may submit to your insurance company. Your Good Faith Estimate reflects what Centered Connections expects to charge you directly. It is separate from any reimbursement you may receive from your insurer.
